Black Hawk pilot Rebecca Lobach failed to follow her flight instructor in the moments before colliding with a civilian airliner over Washington, D.C., according to a new report.

A Delta Air Lines flight narrowly avoided a collision with a U.S. military jet shortly after takeoff from Washington National Airport on Friday.
